An action role-playing survival horror game with tank controls and fixed camera views, featuring real-time battles against visible enemies that can be targeted by cycling through lockable targets within range. Aya must rebuild her abilities by spending experience points to unlock and upgrade powers across four elements—fire, water, wind, and earth—each with three abilities in three tiers. Set nearly three years after the first game, Aya Brea, an FBI agent in the Mitochondrial Investigation and Suppression Team, investigates Neo-Mitochondrial Creature sightings at the Akropolis Tower in Los Angeles, where she encounters the human-passing NMC known as No. 9, and is later sent to the desert town of Dryfield, Nevada. There she teams up with survivalist Gary Douglas and private investigator Kyle Madigan to explore an underground facility called the Shelter, where she uncovers the Second Neoteny Plan—a project to create Artificial Neo-Mitochondrial Creatures using her own genetic material, obtained through secret ties between MIST and the conspiracy, culminating in a massive cocoon that will produce an ANMC capable of releasing a virus to transform all of humanity.
